Job Summary Provides patient services under supervision of a physician. Elicits detailed patient histories, performs physical examinations, gives treatment, prescribes certain drugs and counsels patients on health problems. May suture minor wounds, apply and remove splints and perform other therapeutic procedures. Adhere to HonorHealth credentials Allied Health scope of practice.
Percentage of time spent performing duties varies according to physician and office needs.
Demonstrates clinical competency in primary care settings by providing comprehensive history and physicals examinations with review of systems appropriate to primary care patients in accordance with evidence based treatment guidelines in a Patient Centered Medical Home model.
Manages ordering and review of diagnostic test results as appropriate, under the supervision of a physician. Interacts with patients and families to keep them informed and engaged in the decisions and direction of their health care
Provides counseling education and resource information on wellness, prevention of illness and health maintenance. In partnership with the Physician, refers patients as appropriate for specialty care services and educational training as appropriate for their conditions.
Utilizes disease registries to identify gaps and out of range values in care delivery to utilize support services and care managers to improve metrics of healthcare status
Participates in process improvement to deliver same day appointments and extended hours of availability. Works with the process team to improve efficiencies of care
Maintains, monitors and ensures accurate and timely documentation of patient encounter records to meet criteria set forth by HonorHealth Medical Group coding guidelines, CMS, HIPAA and other governmental regulations.
Acts in a fiscally responsible manner. Strives to deliver value (quality/cost) to the patient by giving the right care by the right individual at the right time.
Participates in clinical integration within HonorHealth Medical Group including process improvement, working with care managers, and coordinating transition of care to specialists and inpatient services. Provide timely follow up post-inpatient discharge for transition of care to the outpatient setting. Monitors patient outcomes relevant to interventions, patients presenting for follow up care either routine or unplanned by managing interventions appropriately.
